    [email protected] is a self-contained independent nursing facility that specializes in sub-acute care and rehabilitation. Our goal is to help our patients achieve their highest level of independence as quickly as possible.

    Our multi-professional team consists of general practitioners, physiotherapists, occupational therapists, speech therapists, dietitians, a social worker and a psychologist. Together they follow an interdisciplinary approach to ensure that all our patients receive high quality, comprehensive care.

    Patients admitted to [email protected] should be stable and should not require highly technical treatment or surgery. The scope of medical care will typically cover post-operative, restorative and medical transitional, chronic or long term care patients.

    Most medical aids pay for admission to a sub-acute facility subject to pre-authorisation. Sub-acute facilities are licenced by the department of health and have to obtain a practice number from BHF for medical aid acceptance.

    A Goal Oriented Multi-disciplinary Approach

    Within 48 hours of admission, patients are evaluated by our skilled multi-disciplinary team. Once the initial assessment is complete, the team develops an individualized treatment plan, sets rehabilitation goals and establishes a projected discharge date.     The patient’s multi-disciplinary team holds weekly meetings to discuss and evaluate the patient’s progress and ongoing care. Family members are encouraged to be active participants in the rehabilitation process – They are invited to attend therapy sessions and formalized training classes in preparation for the care of the patient upon discharge.

    During the team meetings, family members are encouraged to ask questions and express any concerns they may have about their loved ones progress, discharge and at-home care needs.

    Rehabilitation Services Explained

    Rehabilitation programs usually distinguish between brain injury and stroke rehabilitation, as recovery from traumatic brain injury tends to be more prolonged and unpredictable than from non-traumatic brain disease. Other types of brain injury may be included based on age, disability profile and complexity of deficits.

    Brain and Spinal Cord Injury:
    These patients are primarily individuals with traumatically acquired brain or spinal cord damage.

    Individuals who have experience brain injury are likely to have difficulty with memory, communication, thinking and reasoning skills as well as problems maintaining appropriate behavior in social situations.

    Stroke Rehabilitation:
    Individuals who have experience strokes or neurological disorders are likely to have medical problems that include trouble with mobility, swallowing, language and communication, and performing of daily living skills.

    These patients often require significant family support after discharge.

    The principal goals of stroke rehabilitation include maximising the level of independent functioning, preventing and managing complications, facilitating re-entry into the community and optimising psycho-social adaptation.

    The overall objectives are to improve quality of life by enhancing functional capacity and to facilitate discharge to a less rehabilitation-intensive setting.

    The goal of rehabilitation is to help these individuals reach their highest level of independence as quickly as possible in order for them to return to productive living in their homes and communities.

    This is accomplished through individualized programs by the multi-disciplinary team.
